Welcome to Vintage on the Ponds, a 70-bed, licensed Community-Based Residential Facility (CBRF) that has been proudly family-owned and operated since 1993. Nestled on 116 beautiful acres, our community is surrounded by peaceful woodlands, vibrant flowers, and abundant wildlife—all overlooking two serene ponds. Our residents and their loved ones enjoy the outdoors in one of our three landscaped courtyards, perfect for visits and quiet moments alike.

All rooms at Vintage on the Ponds are private and thoughtfully designed. Each room varies in size, décor, and bathroom arrangement to accommodate individual preferences and needs.

We offer 24-hour nursing supervision and a highly qualified, compassionate support staff. Our team takes great pride in delivering exceptional care, ensuring that every resident feels comfortable, respected, and truly at home. We serve individuals across a wide spectrum of care needs—from those who are fully independent to those who require total assistance with daily living. No matter the level of support, dignity and respect are at the heart of everything we do.

Our vibrant activities program helps residents stay engaged with a variety of daily opportunities for enrichment, socialization, and fun. Outings are made easy with our wheelchair-accessible van, and we also offer transportation to and from medical appointments.

Vintage on the Ponds proudly provides respite care, hospice, and palliative services. We are contracted with multiple Managed Care Organizations (MCOs), including Community Care Inc., Inclusa, and My Choice Wisconsin.
