Assisted Living
At Vintage on the Ponds, our assisted living services are designed to support residents who need help with daily activities while still maintaining as much independence as possible. As a licensed Community-Based Residential Facility (CBRF), we provide personalized care in a warm, home-like setting where dignity, respect, and comfort come first.
Our compassionate staff is available 24/7 to assist with tasks such as bathing, dressing, medication management, and mobility. Each resident has a private room and access to inviting common areas, three outdoor courtyards, and beautiful views of our surrounding ponds and wildlife.
Whether a resident needs minimal support or more comprehensive assistance, we tailor our care to meet their individual needs—helping them live safely, confidently, and with a high quality of life.
RESPITE CARE
The Vintage offers respite care for individuals whose dedicated caregivers need a break to rest and rejuvenate. This could give the caregiver a day, weekend, week, or up to 28 days, for personal time. Do you care for a loved one and need a day or two to do things for yourself? Does your situation require you to return to work? Let us help by providing respite care, so it is possible for you to enjoy some free time and regain the energy and vitality needed to care lovingly and adequately for your loved one. The rates for respite stay are provided, for your convenience, in this folder on our rate sheet. Respite care is provided on a first come, first served basis and when room availability allows.We are also able to provide respite care for individuals in need of rehabilitation services. Vintage on the Ponds is a comfortable, home-like environment unlike some hospital or institutional settings. We are equipped to provide the same intense therapy as other facilities, such as skilled nursing homes, while you enjoy the country setting, beautiful grounds and privacy of your own room. HOSPICE CARE
Hospice services are provided at The Vintage through several highly regarded companies. When a loved one is in need of 24-hour palliative or end-of-life care, our staff of professionals will partner with the Hospice group of your choice to assist. Your loved one is able to remain in the privacy of their own room while receiving collaborative care. Your family can relax knowing that our nurses are available to your loved one when needed. Hospice care is available to those who reside at The Vintage or who are guests on a limited respite stay. Hospice staff also provide support services to the family including medical, social, and religious support. The Hospice teams and other staff have been working together for years. We strive to ensure the final road of one's life is comfortable with dignity and respect. Families are welcome day and night, and are encouraged to stay with their loved one.
On-Site Physical Therapy
Please contact the Vintage to see the provided information for Greenfield Rehabilitation Agency. These services enable residents, who require intense therapy, to return from the hospital to their home at The Vintage without having to spend an extended period of time in a skilled nursing facility. Greenfield Rehabilitation Agency is Medicare certified.
Palliative Care
At Vintage on the Ponds, we provide compassionate palliative care focused on comfort, dignity, and quality of life. Our team works closely with residents, families, and healthcare providers to manage symptoms, ease pain, and offer emotional and spiritual support during serious illness.
Palliative care is available alongside ongoing treatments and is tailored to meet each resident’s individual needs. Our goal is to ensure every person in our care feels supported, respected, and as comfortable as possible—every step of the way.
ALZHEIMER'S - DEMENTIA UNIT
We offer a dedicated 15-bed memory care unit designed specifically for individuals in the later stages of memory loss. This secure and cozy environment provides a comforting, home-like setting where residents can feel safe and supported. With a staff-to-resident ratio of 1 to 4, our team is able to provide personalized, attentive care throughout the day.
Our day and dining rooms are always supervised by compassionate, well-trained staff who are specially certified in dementia care and behavior management. Residents enjoy a variety of meaningful activities tailored to their needs and abilities, helping to promote engagement and preserve a sense of purpose.
Surrounded by the beauty of our grounds, residents benefit not only from excellent care but also from a peaceful environment that enhances their quality of life. As residents near the end of life’s journey, we work closely with Hospice providers to ensure they are able to remain in the comfort of their memory care home.
