Vintage On The Ponds is a privately owned, 70-bed community-based residential facility (CBRF), located in Delavan Wisconsin. We offer 24-hour nurse supervision by happy and qualified staff that enjoy their job. They realize that when they are at work, they are in the homes of each and every one of the residents at Vintage On The Ponds.
We take pride in their home, respect those who live here, and maintain our goal, which is to provide the highest quality of life for our residents.
We are situated on 116 acres overlooking two large ponds and are amongst wildlife, trees, and flowers. We have three enclosed outdoor courtyards for our residents to enjoy with family and friends. A small, wheelchair accessible bus is available to accommodate those who enjoy getting out for activities, and we provide transportation to and from doctor appointments. The level of care we are able to provide ranges from care for independent residents to care for those who are in need of total assistance.
ADMINISTRATION
Our administrator at Vintage on the Ponds is always available for tours, admissions, and to address and resolve any issues that may arise. Only on rare occasions is an administrator unavailable to family members. Their constant presence is also convenient for families seeking placement. We encourage families to tour several facilities and to compare them to ours in order to find the best match for their loved one.
ALZHEIMER'S - DEMENTIA UNIT
We offer a separate 17-bed unit for persons suffering from late stages of memory loss. This unit has a separate manager directly overseeing the seventeen residents and a 1 to 3-4 staff to resident ratio. This unit was designed to be small and cozy to give the residents a feeling of safety and security. The day room and the dining room are never left unattended, for we offer activities throughout the day. There is also a weekly outing where residents are shuttled to and from a planned day trip. The beautiful grounds only add to these residents' quality of life. The staff on this unit are required to have additional training and have chosen to work with this specific client group. At the end stages of life, these residents are able to remain on this unit (their home) with the assistance of Hospice services.
ON SITE PHYSICAL OCCUPATIONAL & SPEECH THERAPY SERVICES
Please see the services that Genesis offers (included on this site). This is a unique service enabling residents who require intense rehabilitation to return from the hospital. Genesis is Medicare certified so therefore many of their services are covered.
RATES
Our facility accepts long term care insurance or private pay. All of our rooms are private and vary in size and decor. Rates are monthly. A level of care will be added accordingly to the room rates listed below. Our rates range in price as follows:
$2700.00 - $3050.00 shared bathroom with no shower
$3375.00 private bathroom with no shower and patio
$3275.00 private bathroom with tub or shower
$3675.00 - $5700.00 Suites, private bathroom with shower, patio, some with kitchen (Perfect for the resident that is moving from their own home or apartment. Great for a couple.)
Alzheimer's - Dementia Unit Rates $3375 - $3800 Shared and Private rooms.
Respite rates vary from $180 - $220 daily

RESPITE CARE
Vintage On The Ponds offers respite care. Respite care is to provide a break for caregivers, whether it is family members, volunteers, or paid staff working in a private home providing care. This can mean a day, weekend, week, or up to 28 days for whatever the reason. Do you care for a loved one and need a day or two to do things for yourself, or do you need to go back to work? Let us help by providing respite care so that it is possible for you to enjoy your life again as well.
We are also able to provide respite care for persons in need of rehabilitation. Vintage On The Ponds is a homelike environment unlike some hospital or institutional settings. We are able to provide the same intense therapy as other facilities such as a skilled nursing home, while you enjoy the beautiful grounds and privacy of your own room. Genesis Rehabilitation Services is Medicare certified.
HOSPICE CARE
Vintage On The Ponds hospice care is provided by the Visiting Nurse Association. When a loved one is in need of 24-hour care, we are here to help. Your loved one is able to enjoy the privacy of their own room while receiving care from qualified staff and the comfort of knowing that our nurse is available to them when needed. Hospice care is available to those who reside at the Vintage On The Ponds or to persons currently in their own home who require care that can no longer be provided for them at home.
Hospice, our nurses, and staff have been working together for years to make sure that the end stages of a person’s life continue to be filled with dignity, quality, and respect. Families are welcome at all hours, day and night, and are allowed to stay with their loved one if they wish to. We also have accommodations for those who are here from out of town. This guest room (located on the upper level) includes a full bathroom and the privacy that may be needed during this sad and stressful period. Hospice care is covered by Medicare.
OUTPATIENT THERAPY SERVICES
Genesis Rehabilitation Service provides comprehensive Outpatient Therapy Services at Vintage On The Ponds. Professional Physical, Occupational, and Speech Therapy are available to the Delavan area community. A fully equipped therapy center is available inside the Vintage On The Ponds facility. Outpatient parking and a wheelchair accessible entrance are available as well.
